REGISTRATION INSTRUCTIONS

Please complete the Registration Questionnaire of the camp you are interested in attending. After completing the Registration Questionnaire press Submit at the bottom of the form and all of your information will enter into our data base. 

Once you have completed the questionnaire, please complete the Medical Waiver Form. 

After completing the Medical Waiver Form, mail the Medical Waiver Form, Cash or Check (made out to the University of Portland) with any coupons to the mailing address below:

Camps have limited space, so please finalize your registration by completing the registration questionnaire and mailing your amount due to the University of Portland. Your registration is not complete until payment is received.
